GIFT CARD REGISTRATION FORM

State of Florida rules which govern the use of Gift Cards make it impossible for us to replace a card which is lost or stolen since they prohibit us from expiring a Gift Card.  A Gift Card must be physically presented for use and the possessor of the card is the final arbiter of any usage or dispute but by completing this registration form, we can issue a replacement for a lost or stolen card based on this purchaser information.  This registration information, however, must be completed within 7 (seven) days of the receipt of our confirmation notice to be valid for this purpose.
